EMG/electromyography/electromyographic examination

Electromyography is a supportive diagnostic method used to evaluate a wide range of neuromuscular disorders — conditions affecting the function of nerves, muscles or neuromuscular transmission.

EMG consists of two parts:

  • nerve conduction studies (NCS)

  • needle EMG, performed when clinically indicated.

Nerve conduction studies (NCS)

Nerve conduction studies are performed by applying small electrical impulses through surface electrodes placed on the limbs. By recording the responses, the physician can determine whether there is damage to a peripheral nerve or nerve root (commonly used to confirm carpal tunnel syndrome and assess the need for surgery), peripheral nerve dysfunction (e.g., diabetic polyneuropathy), nerve injury after trauma and other conditions.

The electrical stimulation may feel uncomfortable or mildly painful for some patients, and brief muscle twitches may occur during stimulation.

Needle EMG

Needle EMG involves inserting a thin needle electrode into the muscle to record bioelectrical activity generated by muscle fibres. This part of the examination may be uncomfortable due to the insertion of the needle.

Preparation before the examination

  • Do not apply creams, lotions or any other products to your limbs on the morning of the examination (to minimise skin resistance).

  • Inform the nurse or physician if you have a pacemaker, cochlear implant, or if you are taking blood‑thinning medication (to assess possible contraindications or adjust the extent of the examination).

  • If you tend to faint during blood draws, please inform us in advance.

The EMG examination is performed jointly by a nurse and a neurologist. The findings are evaluated by the neurologist.

1. Does the EMG examination hurt?

The examination may be perceived as uncomfortable, but for most patients, it is not outright painful. It consists of two parts:

  • Stimulation EMG (Nerve Conduction Studies): Using a small surface electrode, we send a mild electrical impulse into your nerve. Patients most frequently describe this as a quick tingling, pins and needles, or a twitch in the muscle (similar to static electricity).
  • Needle EMG: We use extremely thin, disposable needles. You will feel the insertion similarly to a routine vaccination or a blood draw.

My nurse and I guide the entire process with maximum sensitivity and regard for your comfort.

2. How long does the examination take?

The duration of the examination depends on the scope (whether we are examining one hand for carpal tunnel syndrome, or both legs for polyneuropathy) and whether we perform only the stimulation EMG (nerve conduction studies) or also the needle EMG. Standardly, the entire visit—including paperwork and evaluation of the results—takes between 20 and 45 minutes.

3. Do I need to prepare for the examination in any special way?

No complex preparation is necessary. You can eat, drink, and take your regular medications as usual.

Important: On the day of the examination, do not apply any body creams, lotions, or oils to your body (especially to your hands and feet). The skin must be clean and dry for the electrodes to properly record the signals.In the winter months, we recommend arriving a bit earlier so that your limbs can warm up to room temperature in the waiting room (cold can distort the measurement results).

4. Is a needle EMG "gentler" than a nerve conduction study (NCS)?

Patients often ask if a needle EMG is gentler or less painful than a conduction study. However, these are not two versions of the same thing—they are two entirely different examination methods, and each looks at something completely distinct.Think of it like a mechanic diagnosing a car:

  • Nerve Conduction Studies (NCS): This method tests the electrical wiring (the nerves). Using mild electrical impulses on the skin, we measure how fast and well your cables transmit signals. 
  • Needle EMG: This method tests the engine performance (the muscles). A very thin needle is inserted directly into the muscle to listen to its electrical activity, both at rest and during contraction. No electricity is sent through the needle into your body—the needle acts purely as a microphone recording the muscle's sounds.

Summary: We cannot choose a "gentler" option because one method cannot replace the other. To get a complete picture of your health, we often need to examine both the cables (conduction) and the engine (needle EMG).
